Diabetes is the 5th leading cause of death in Asian Americans and South Asians have a higher prevalence of diabetes compared to Whites, Hispanics, and African Americans.

The national rate of diabetes is 7%. Research shows as many as 23% of South Asians have diabetes and approximately 18% of Arab Americans.

The All One Community program is a family-centered diabetes education program designed to help participants adopt healthy behaviors related to diabetes prevention and management. The program is tailored for South Asian and Arab community members in Chicago and is available in English, Urdu, Hindi, and Arabic.

The program consists of 13 sessions with a trained community health educator that will provide information about diabetes and healthy behavior changes, along with activities and goal setting to support you to make the changes to improve your health! The program is free, and you will receive supplemental items to support your goals, such as exercise bands and a water bottle.

As part of the program, you will be required to participate in a screening every 3 months, where your blood will be drawn to test your blood sugar levels. A total of 4 blood draws will occur during the program.
